Release of Liability/Assumption of Risk 2024-2025
In consideration of being allowed to participate in the South Lake Home Educators Support Group program:
I acknowledge that all activities in this program are strictly voluntary. I choose to participate, and allow my child(ren) to participate, to the extent I deem appropriate, considering our physical health, abilities, and medical conditions. I understand and agree to be supported in my choices and to support others in theirs. I also understand that teens driving themselves to and from the program do so voluntarily, with their parents' support.
I recognize that South Lake Home Educators' volunteers adhere to high safety standards and that safety procedures and rules will be communicated before each event. I agree to follow all safety guidelines. If I do not understand the risks or safety procedures, I will ask for further clarification.
I knowingly and freely assume all such risks, known and unknown, even if arising from the negligence of others, and accept full responsibility for my participation.
I understand it is my responsibility to inform South Lake Home Educators' volunteers of any physical limitations, medical conditions, or injuries, including but not limited to heart conditions, neck or back problems, recent surgeries, pregnancy, or any other potential issues. In case of an accident or illness, I understand that a South Lake Home Educators volunteer will provide basic first aid and arrange for medical services if needed.
I acknowledge that South Lake Home Educators (SLHE) and if attending the co-op, the West Orange Church of Christ (WOCC), shall not be held responsible or liable for any bodily injury, illness (mental or physical), or property damage or loss resulting from my negligence. I agree to hold SLHE and WOCC harmless for any damages, illnesses, or injuries incurred during co-op hours or SLHE-sponsored activities/events on the co-op (WOCC) premises.
This agreement serves as a release and assumption of risk for me and all members of my family while participating in the program or traveling to and from the program. |
